Care Programs
Post-Hospital & Transitional Support
The first thirty days home are the ones that matter most.
Discharge day feels like the end of something. For families, it's usually the start of the hardest month — new instructions, new equipment, follow-up appointments, and a person who is weaker than anyone expected.
We cover the gap between the hospital and normal life.
What we handle
- Preparing the home before discharge — clearing paths, setting up the bedroom, stocking the kitchen
- Transportation home and to every follow-up appointment
- Medication reminders on the new schedule
- Meal preparation aligned to any dietary guidance from the care team
- Encouragement with prescribed activity and movement
- Support with bathing, dressing, and mobility while strength returns
- Watching for changes and alerting family promptly
- Light housekeeping so recovery isn't competing with chores
Coordination
We work alongside home health, therapy providers, and physicians — we handle the non-medical hours they don't cover.
Fast starts
We can often begin care within 24 hours of a discharge call. If you're standing in a hospital hallway right now, call us.
Let's take the next step together.
Whether you need care starting tomorrow or you're just beginning to look, we'll give you a straight answer about what's possible — including if the answer isn't us.
